I thought you meant you were using a different brand of the NEC chipset PCI-E USB card since your Gigabyte GA-X99-UD4P has no PCI slots. So you are not using any USB cards at all and just the onboard Intel for USB 2.0? Without NUSB 3.5 drivers loaded were the on board rear USB 2.0 ports working properly for USB mouse and USB keyboard?
