Please click on Start button and than rightclick Computer and choose Properties.
Click on Advanced System Settings and on the Advanced tab click on Settings under Startup and Recovery.
Now disable the checkbox Automatically reboot.
Now you will be able to read the blue screen.

What does the blue screen tell you exactly and what OS you're running. Are you using an original install.wim or do you use one that was captured or modified before?

You just need to copy the install.wim from your Vista DVD \sources directory to a directory on your HD. Than you put the location of this directory to the line:
SET vpath=%CD%
for example:
SET vpath=D:\VistaDVD
That requires that you've copied the Vista DVD to the VistaDVD folder on your D: drive (D:\VistaDVD).
All the rest is done by the batch.

Before redoing everything... just to make sure, that we're talking about the same overview window: I've no picture present ATM, but my is a little window, were it shows the Apps wich supposed to be installed - listed by their description. In your case it would look like this:
OPERA_9.21_ENG

Once it has finished installing an app it will be checked with a green checkmark in front of it. In background the cmd's are popping up and - depending on the app, that's installed - I see the one or other progress bar windows popping up.
If you don't see this little window, than you should try to redo everything.

I don't see any errors inside your xml, which could causing your problem. What kind of DVD you're using. Is it may be an OEM DVD?
Or has it been modified using vlite?

The cmd /c command is necessary to get it worked with %Appsroot% environment variable. Without cmd /c it would not install directly from media and this is at least causing the cmd windows, so you can't get rid of these screens - or better I would not know how to get rid of it.
But if you install in auditPass - like you do - you will get the overview list. I guess you don't get such list for the moment, cause you don't use a Description. Please fill out the Description tags (<Description></Description>) and you should get the overview window again - but you'll still have the poping up cmd boxes in background.

You can use setx command to save variables to system envrionment. setx is included in Vista. For XP you need to extract setx from MS Support Tools for XP Service Pack 2.
Syntax:
setx variable value (-m)
-m is optional, it will set the variable to machine environment.
Without -m it's set to users environment.

OK... we need to check now, if your registry tweaks are applied at all. So please remove the following lines from your Autounattend.xml:

If the tweaks are not applied, it means that there is something wrong with the path to your .reg file. If they would be applied, than please let me know.
BTW, do you use an original Ultimate version or was it captured before or edited using vlite?

Thanks for the feedback. BTW I would never recommend to replace unattend.xml inside Panther dir/subdir. This can end up very confusing and is not recommended by MS, too.

Your system seems to have problems, if you don't set the refresh rate. So please set the refresh rate. BTW my system is comming up with the same error if I set the refresh rate inside xml file. So I'm forced not to set the refresh rate.

I suspect one of your installation cmds causing a delay which is causing the flush. I could imagine that one of your cmds is still running while setup is trying to execute sysprep.exe /generalize. Could you please verfiy that.

EDIT:
The Flush could be caused by some hardware, which is not installed during setup, too. Do you may use some hardware, which Vista does not detect/install the drivers for? Than remove it for test or if you can't remove install the drivers during setup.
If this still does not help out you should recreate both xml files "manually" using WSIM.

Did a test install with your Autounattend.xml and it was not working for me, too. Have not figured out why yet but I've a workaround I was using before:
You have to use two answer files. The first answer file will process evrything till auditUser pass. Using sysprep generaliz a second xml is called, which will do the rest:

AFAIK it does slipstream the updates / hotfixes. It's normal behaviour of install.wim that it's growing, once you change something. If you want to reduce the size of install.wim after doing changes, you need to export install.wim to a new .wim file.

Well, what you're doing with your set_dirs.cmd is done by perpelab.cmd of my Vista Batch Modules. Have a look at the build_menu.cmd, which is creating the menu by install.wim index. Thats more flexible instead fo checking for "Business", "Ultimate" etc..

I don't wanna bother, but the "some numbers" would be helpfull, too.
NTFS.SYS could mean that it's something with the file system, and I would guess it has something to do with the filter driver used to mount images... but not for sure...

If you've Antivirus Software installed on your Vista, please disable it and than try again.
