are there any issues with installing this over an already patched i386?

"already patched i386"? what do you mean? anyway replacing your files with XPize ones you will lost all patches those files had. I mean, if you have patched shell32.dll with custom icons and them you replace it with XPize one...of course you will lose your previous customization of the file. Is that what you mean?

i was asking if i could just do 1.1 over my innitial 1.0 patch or if i need to use my original files and patch them from scrach

Oh, ok :P Well, it is always better to patch them from scratch, because from version to version new files are added or removed in XPize.
