Not too sure on this one,

Try going into folder options then go to File Types, you should see all of the extensions in there find yours and go to advanced and put a new one into there. Is that what you mean?

Let me know if it works or if this is what you want to do

I know how to do it the tools>folder options way but how can you add it into an unattended install? I added the context menu and then in the registry I looked in hkcr\ but couldn't find anything.

Not too sure on unattended installs i only use ghost.

You might be able to build it into the install or a system folder or write a script that will add it in after the install like run the script using sysprep.
