Icon pack issue, shortcut ontop of shortcut.. help ?

I have made a nlite of Windows XP Pro SP2 with some addons and such, I just have an issue with Vista Icon Pack v3 or v2. When I install everything is well, other than the fact if I drag a shortcut onto the desktop or if I create one a weird folder icon is over top the icon which is suppose to show ?? I can post a pic you cannot imagine what this looks like

I installed my regular XP Pro CD in VMWare and I did not have the issue, it could be caused by included hotfixes, included programs, something needed which was ripped out using nlite ??

I have heard of people having this is issue

I included this add on HERE which is the Vista icon pack and I got the issue

I also installed Vista icon packv3 from fresh install with the same issue
