Definitive Solution To Corrupt Usbehci.sys?

Someone PLEASE post a definitive, clear solution to the corrupt usbehci.sys file that many are experiencing when booting from their CD. I've seen so many posts, but many contradict one another. Anyone that's had success willing to share?

I had the same problem 10 minutes ago.

I had the file usbehci.sy_ in lowercase. it should be in UPPERCASE. After renaming it uppercase it worked.

I think it could be a good idea to make all files in the I386 folder uppercase.
