Does anyone know how to create a Word document, from scratch, like the one I have attached? I want to create fields that users can fill in and submit, just like in the attached document.

I don't know in Word 2007 but in 2003 you use the Forms Toolbar and insert a Text Form Field

Then you can protect the document setting the editing restrictions to Filling in forms.

Hope this translates to something in '07
