DCPROMO did not request a Recovery password

I just created a W2k3 CD with SP1 slipstreamed and the install looked good until I promoted the server to the first DC. During the process, I was not prompted for a recovery password. After the reboot I got the "DSRestore filter failed to connect to the local SAM server" error. I was able to reset the Recovery password, but I am wondering why I didn't get prompted during the promotion. Any ideas?
