Index Weirdness

Take a look at the excerpt below from my INDEX. All items are marked properly and appearing in the index, but only the items before page 80 are sorting themselves into the SETUP category properly, the rest just appear below in chronological order, TOC style. Pretty useless. None of the other sections display this behavior. Any ideas why or how I can make it stop?

Anyone? Are there some hidden limitations to how many items will sort themselves under one main index heading? Will certain section breaks cause the index entries to sort out individually as well? Anyone?
