MS Word selectable calendar

Does anyone have a selectable calendar in a drop-down field already programmed/created that they could share with me?

I'd like to be able to show expected order date and expected ship date in an MS Word 2000 document, and I know nothing about VBA to hide the calendar after a selection is made and leave the selected date in a form field.
