OK, so our setup is as follows:

3 Vlans

Vlan 102 - 192.168.100.0/24

Vlan 103 - 192.168.101.0/24

Vlan 104 - 192.168.102.0/24

DHCP server sits on VLAN 102

The problem is:

When a PC is plugged into a port which is on Vlan 104 it should get an address in the 192.168.102.0/24 range but in fact it is getting an address in the 192.168.100.0/24 range.

Each of the Vlans are configured on the switch and they are all configured with ip-helper addresses to act as a relay agent for the dhcp server.
