Microsoft's own Regclean. It's free.

I use RegSeeker, freeware. Digs up tons of bad/suspect entries and marks them green (safe to delete) or red (needs investigation).

Interestingly, this cleaner impressed the author of a shareware reg cleaner (RegSupreme) in a comparison test of other reg cleaners he did a few years ago.
